DRE supplemental request in DWI case
1. copies of DRE officer’s Preliminary Training and DRE School Certificates
2. copy of his DRE Certification card
3. a copy of his DRE Rolling Log
4. a copy of his IACP DRE certification progress log. (this is a one-page document that is handed out to DRE's
at the beginning of their training and charts their progress through each phase of training, included field cert's.
5. copies of all training evaluations conducted or witnessed by the DRE as part of his field cert training.(during field cert's
the DRE must complete a minimum of 12 evaluations on volunteer subjects who are "believed" to be under the influence)

(1) copies of all drug evaluations
(2) evaluation logs
(3) resume
(4) certification and recertification progress logs
(5) certificates
(6) instructor ratings and summaries of student critiques
(7) records of classes taught by each instructor
(8) file certified DREs and instructors
